ישמרך comes from ברכת הכהנים, Birkat Kohanim (the Priestly Blessing),
a blessing that is also said over the children every Shabbat evening at the dinner table:
 
יְבָרֶכְךָ ה׳ וְיִשְׁמְרֶךָ
May G-d bless you and protect you.
יָאֵר ה׳ פָּנָיו אֵלֶיךָ וִיחֻנֶּךָּ
May G-ds light face you and educate you.
יִשָּׂא ה׳ פָּנָיו אֵלֶיךָ וְיָשֵׂם לְךָ שָׁלוֹם
May G-ds face carry you and grant you peace.
 
The protection is not only physical,
but also the safeguarding of blessing itself.
Our sages explain that blessings need to be protected,
and it is said that when God blesses, He also protects.
